Job summary

University Health Network is seeking a Social Worker for the General Internal Medicine team at Toronto Western Hospital. The role focuses on assessing patients, developing intervention plans, and coordinating with families and the interprofessional team to support meaningful changes.

You will manage Alternate Level of Care placements and contribute to education and research activities. Requirements include an MSW and registration with the Ontario College of Social Workers and Social Service

Qualifications

• Master of Social Work degree required.
• Registered with Ontario College of Social Workers and Social Service Workers.
• Membership in Ontario Association of Social Workers preferred.
• Minimum three years in an inter-professional hospital team; acute care experience preferred.
• Strong verbal and written communication skills.
• Excellent interpersonal, time management and organizational abilities.
• Research experience and student supervision are assets.

Responsibilities

• Assessing patients to determine needs and planning interventions.
• Coordinate with families and interprofessional team; lead meetings.
• Update and adjust care plans; liaise with community agencies.
• Complete Alternate Level of Care (ALC) applications and communications.
• Provide clinical teaching to students and staff; participate in research.
