Job summary

The University Health Network (UHN) at Toronto General Hospital is seeking a temporary full-time Social Worker to support the Transplant program for 12 months. The role involves ambulatory care and collaboration with an inter-professional team.

You will assess patients, develop and implement intervention plans, liaise with community agencies, and contribute to teaching and research. Requirements include a MSW, provincial registration, and CKD experience.

Qualifications

• Masters in Social Work required
• Registration with the Ontario College of Social Workers and Social Service Workers required
• Experience in ambulatory care is required
• Experience working with chronic kidney disease populations is required
• Excellent verbal and written communications skills

Position Summary

Working under the direction of the Manager and in collaboration with other members of the interdisciplinary team, this temporary full-time outpatient social work position will support the Transplant program at Toronto General Hospital (additional areas and duties may be assigned based on departmental needs).

Duties

• Assessing patients for the purpose of evaluating the need for service, the nature of the problem, and formulating recommendations or treatment plans
• Implementing an intervention plan with patients, family members, significant others, and theinter-professional team for the purpose of facilitating changes in behavior, attitudes, feelings and/or the environment
• Reassessing and modifying the intervention plan as required
• Liaising with appropriate community agencies
• Completing applications, establishing contact with members of theinter-professional team and community agencies to obtain, provide or exchange information relating to patients' psycho-social functioning and health care in the past, present and future
• Rendering services for the immediate benefit of the Hospital and the community
• Providing clinical teaching to Social Work Students, Medical Students, Professional Colleagues at University Health Network, Faculties of Social Work and in the community
• Participating in relevant research activities, projects, etc.
• Performing activities required for the effective and efficient operation/maintenance of the Social Work professional group

Qualifications

• Completion of a Masters Degree in Social Work, required
• Registration with the Ontario College of Social Workers and Social Service Workers, required
• Membership in the Ontario Association of Social Workers, preferred
• Three (3) years experience working with an inter-professionalteam in a hospital setting, preferred
• Experience in ambulatory care, required
• Experience working withchronic kidney disease populations, required
• Experience working with individuals with complex chronic health conditions, an asset
• Experience working with geriatric populations, an asset
• Excellent verbal and written communications skills
• Excellent interpersonal, time management and organizational skills
• Excellent problem solving skills
• Research experience/education an asset
• Knowledge of policies, legislation, program's and issues related to social welfare institutions and services
• Knowledge of a second language an asset
