Job summary

University Health Network (UHN) seeks a Pharmacist for the Transplant Outpatient Pharmacy at Toronto General Hospital. You will review prescriptions, monitor therapies for safety and effectiveness, and support medication reconciliation in a fast-paced, multidisciplinary setting.

The role requires a PharmD or BSc in Pharmacy, Ontario registration, and certification. Temporary full-time, 12-month contract with competitive hourly pay and comprehensive benefits.

Qualifications

• University degree in pharmacy is required.
• Active registration with the Ontario College of Pharmacists (Part A) is required.
• Certificate of competency from the Pharmacy Examination Board of Canada is required.
• Experience in hospital outpatient pharmacy and multi-disciplinary teams is preferred.
• Strong communication and customer service skills are essential.

Responsibilities

• Review, verify, check and dispense all prescribed medication orders for appropriateness.
• Identify, resolve and prevent drug-related problems.
• Monitor drug therapy for effectiveness, toxicity and guideline adherence.
• Conduct medication reconciliation and follow-up on incidents.
• Respond to drug information questions and supervise staff as needed.
• Work with a multidisciplinary team to ensure patient safety and service quality.
