Job summary

University Health Network (UHN) in Toronto Rehab Bickle Centre is seeking a Social Worker for a casual, on-site role. The position supports the Complex Continuing Care Program with cross-coverage to other sites, reporting to the Allied Health Manager.

Shifts are 8:00am–4:00pm, on weekdays, with varied hours and a casual status. Primary duties involve assessments, care planning, coordination with the interprofessional team and community agencies, and providing education to students and

Qualifications

• MSW degree required or equivalent.
• Ontario College of Social Workers & Social Service Workers registration.
• Membership in Ontario Association of Social Workers preferred.
• 2–3 years experience with interprofessional healthcare teams preferred.
• Knowledge of policies and legislation relevant to social welfare and health care.
• Research experience/education is an asset.
• Student education/supervision experience is an asset.
• Knowledge of a second language is an asset.

Responsibilities

• Assess patients to evaluate need for services and formulate treatment plans.
• Implement intervention plans with patients, families, and interprofessional team.
• Facilitate family and team meetings.
• Reassess and modify intervention plans as required.
• Liaise with community agencies for information exchange.
• Prepare applications for Alternate Level of Care facilities; coordinate with team and agencies.

Position Summary

Working under the direction of the Allied Health Manager and in collaboration with other members of the interdisciplinary team, this SW position primarily supports the Complex Continuing Care Program, with cross coverage to other sites; however coverage areas may change based on operational needs and requirements.

Duties

• Assessing patients for the purpose of evaluating the need for service, the nature of the problem, and formulating recommendations or treatment plans
• Implementing an intervention plan with patients, family members, significant others, and the interprofessional team for the purpose of facilitating changes in behavior, attitudes, feelings and/or the environment
• Facilitating family/team meetings
• Reassessing and modifying the intervention plan as required
• Liaising with appropriate community agencies
• Completing applications for Alternate Level of Care (ALC) facilities, establishing contact with members of the interprofessional team and community agencies to obtain, provide or exchange information relating to patients' psychosocial functioning and health care in the past, present and future
• Providing clinical teaching to Social Work students, other students, and colleagues
• Participating in relevant research activities, projects, etc.
  
  
  

Qualifications

• Completion of a Masters Degree in Social Work (M.S.W) or equivalent
• Registration with the Ontario College of Social Workers and Social Service Workers
• Membership in the Ontario Association of Social Workers, preferred
• 2 - 3 years experience working with an interprofessional team in a health care setting, preferred
• Knowledge of policies, legislation, programs, issues related to social welfare institutions and services, the Health Care Consent Act, and Advance Care Planning
• Research experience /education, an asset
• Student education/supervision, an asset
