Job summary

Trillium Health Partners is seeking a Case Manager for the North Peel Case Management program. The role focuses on supporting individuals with serious mental illness through home visits, community outreach, and collaborative care planning with clients and families.

The incumbent will coordinate care plans, provide skills teaching, and help navigate social and medical services while working with a clinical team. A strong emphasis on communication and independent teamwork is essential.

Qualifications

• Bachelor’s Degree or Community College degree in a health-related discipline.
• Current registration with a recognized college.
• Experience with adults living with serious mental illness.
• Knowledge of psychosocial rehabilitation and recovery philosophy.
• Excellent communication, organization, and problem-solving skills.
• Proven ability to work independently and as part of a team.
• Valid driver's license and access to a vehicle.
• Experience in treatment planning and intervention.

Responsibilities

• Case management support to individuals with a history of serious and persistent mental illness.
• Work in partnership with clients, families and the clinical team.
• Meet clients in their homes, in the community or in the office.
• Set individualized care plans to improve daily living activities.
• Provide skills teaching and facilitate connections to community resources.
• Aid clients with navigating medical and social assistance systems.
