Canadian Mental Health Association | York Region & South Simcoe seeks a permanent Case Manager for the Assertive Community Treatment Team (ACTT) – South West, based in Vaughan. The role supports clients 16+ with serious mental illness or addictions and involves community travel.

Position: Permanent, full-time, working 35 hours per week. Evening hours and weekends may be required.
Salary: $56,444-68,429 per annum, *Candidate experience will be considered when determining the starting salary.

Location: The Case Manager will be working primarily in our Vaughan Office located at 8271 Keele St Unit 2, Vaughan, ON L4K 1Z1 and out in the community and will be required to travel within York Region and South Simcoe to visit clients and travel to clients homes.
The Case Manager works with clients 16 years of age and older who are experiencing significant difficulties in day-to-day and role functioning as a result of serious and persistent mental illness and/or addictions. The Case Manager collaborates with the client to identify strengths and needs and develop long term goals, short term objectives to match these goals, and concrete action steps to achieve the objectives. As part of this process individuals are assisted to acquire and use skills, and to connect with formal and informal supports, peer support, professional services, and resources to promote their recovery, improve their quality of life, and reduce the likelihood of relapse. Other duties as required.
The Case Manager collaborates with clients to provide individualized and flexible services to engage, support, and integrate clients into the community, and facilitate their use of resources and supports to maximize their independence and potential. The Case Manager adheres to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety. The Case Manager participates in individual and systemic advocacy initiatives to improve resource access and availability.
In addition to compliance with all organizational policies and procedures, the incumbent is also expected to behave ethically, demonstrate competence in effective communication and team building, demonstrate effectiveness in working in a collaborative environment, and demonstrate confidence in decision-making and building relationships.

Working Conditions: Desk work up to 7 hours a day in standard shared office environment, meetings clients in community and their homes, extensive travel in York Region and South Simcoe, evening and weekends may be required.
