Job summary

Fraser Health in British Columbia is seeking a Registered Nurse/Registered Psychiatric Nurse to provide assessment, treatment, consultation and follow-up for clients and caregivers within an interprofessional team focused on person-centred and recovery-oriented care.

You will monitor and adjust care plans, administer prescribed psychotropic medications, participate in case conferences, educate staff and families, and liaise with community agencies to coordinate services.

Qualifications

• Current practicing registration as RN and/or RPN with BCCNM.
• Two years recent related experience or equivalent.
• Valid BC Class 5 Driver's license and access to a vehicle.

Responsibilities

• Provides clinical nursing services to clients and caregivers referred to the team.
• Administers prescribed psychotropic meds, monitors effects and ensures compliance.
• Provides education on mental health and substance use to caregivers and colleagues.
• Collaborates with interprofessional team to adjust care plans as needed.
• Coordinates with community services to ensure timely, seamless care.
• Participates in case conferences and supports crisis intervention when required.
• Documents assessments, interventions and outcomes per Fraser Health standards.
