Job summary

Retirement Concepts is seeking an experienced Licensed Practical Nurse (LPN) to lead a care team in a long-term care community in British Columbia. The role involves coordinating care delivery, implementing individualized plans, and ensuring residents’ rights are respected within a holistic wellness framework.

You will liaise with physicians, families, and other care professionals, administer medications, and contribute to interdisciplinary care conferences.

Qualifications

• Current license with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.
• Valid Occupational First Aid Level 1 certification.
• Successful annual RAI competency testing.
• Previous experience in a long-term care facility is an asset.
• All successful applicants must pass the vulnerable sector Criminal Record Check.
• Ability to read, write, and speak English proficiently.
• Computer skills including Word, Excel, Outlook.

Responsibilities

• Be an engaged collaborative team player, developing and maintaining professional relationships.
• Assesses residents’ needs; implements and evaluates individualized resident-centered care plans.
• Ensures residents rights are observed and adheres to holistic wellness philosophy.
• Liaises with physicians, families, and other staff to ensure quality resident-centered care.
• Administers prescribed medications and treatments within regulatory guidelines.
• Monitors and reports changes in residents’ health and notifies families and physicians as needed.
• Active participant in Interdisciplinary Care Conferences and care plan reviews.
