West Coast Seniors Housing Management is seeking a Licensed Practical Nurse (LPN) to lead care delivery in a community setting. The LPN reports to the Director of Care and coordinates care within the resident-centered framework.

The role requires BC LPN licensure, First Aid Level 1, and RAI competency testing; experience in long-term care is an asset. A positive team leader who supports staff development is desired.

Qualificações

• Graduate of an approved Practical Nursing program and licensed with the BC College of Nurses and Midwives (BCCNM).
• First Aid Level 1 certification.
• Successful annual RAI competency testing.
• Long-term care experience is an asset.
• Vulnerable sector Criminal Record Check required by provincial guidelines.

Responsabilidades

• Coordinate delivery of care according to individualized care plans.
• Liaise with physicians, care teams, families and other staff to ensure quality resident-centred care.
• Assess and document changes in residents' conditions and report to RN or designate.
• Administer prescribed medications and treatments within scope of practice.
• Contribute to care plan reviews and interdisciplinary care conferences.
• Support admissions, discharges and confidentiality in community operations.
• Provide orientation and coaching to staff; use on-the-spot education to improve care.
• Maintain knowledge of community policies, emergency procedures and safe work practices.
