Job summary

VON Canada is seeking a Licensed Practical Nurse to join our visiting nursing team. You will deliver holistic, person-centred care in clients’ homes and clinics, with an emphasis on autonomy and patient connection. The role includes an evening/weekend rotation and travel within the community.

Qualifications

• Must be licensed as an LPN in the province of practice.
• Valid driver's license and access to a reliable vehicle are required.
• Willingness to travel for client visits and home-based care.

Responsibilities

• Perform client assessments.
• Provide primary care in collaboration with other healthcare professionals.
• Administer medications and perform wound care.
• Manage chronic diseases and provide diabetes education.
• Collect specimens and participate in rehabilitative programs.
• Deliver palliative care and other nursing duties as needed.

What we're looking for:

• Must be licensed as a Licensed Practical Nurse in the province of practice
• Community health experience is an asset
• Must enjoy driving, have a valid driver's license and have access to a reliable vehicle
• A current and original copy of a satisfactory Vulnerable Sector Search and Child Abuse Registry Check is required
• Must be able to wear Personal Protective Equipment (PPE)
