Job summary

Cogir Senior Living is seeking Licensed Practical Nurses to deliver patient-centered care to residents in our senior living communities. You will collaborate with a multidisciplinary team to implement care plans and ensure high-quality documentation.

The role emphasizes compassion, communication, and adherence to provincial standards, with opportunities for onboarding, training, and growth within a mission-driven organization.

Qualifications

• Current registration as a Licensed Practical Nurse in the applicable province.
• Demonstrated sound clinical knowledge.
• Experience working with seniors; experience with unregulated care providers preferred.
• Education and experience in gerontology or related fields is an asset.
• Strong drive, teamwork and customer service focus.
• Excellent written and oral communication; critical thinking.
• Strong organizational and time management skills.

Responsibilities

• Provide planned, patient-oriented, goal-directed nursing care per provincial standards.
• Collaborate with multi-disciplinary team, communicate with residents, families and staff.
• Implement and evaluate care plans; adjust as required.
• Complete and ensure accurate documentation of resident care assessments and plans.
• Coach and mentor unregulated care providers as needed.

How You Make a Difference

• LPNs are responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.
• LPNs are an integral part of our multi-disciplinary team who build trusted relationships with our residents, families and teams with continuous ongoing and effective communication.
• LPNs implement and evaluate the effectiveness of the care plans and make changes as required.
• LPNs complete and ensure resident’s clinical care assessments, care plans and documentation are accurate, up to date and communicate changes as required.
• LPNs teach, delegate and direct unregulated care providers, providing them with coaching & mentorship.

What You Bring

• Current registration as a Licensed Practical Nurse in the applicable province
• Demonstrated sound clinical knowledge
• Experience working with seniors and with unregulated care providers preferred
• Education and experience in gerontology, rehabilitation, community/complex/long term care an asset
• Strong drive, respect and an understanding of the emotional and physical demands of the role
• Focus on teamwork and customer service with the ability to build trusted relationships
• Strong written and oral communication skills and critical thinking skills
• Demonstrated organizational and time management skills
• Computer proficiency and experience with Microsoft office applications
• Current Basic Cardiac Life Support and First Aid certification required
• Current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
