Job summary

Cogir Senior Living in Ajax, Ontario is seeking a Registered Practical Nurse (RPN) to join our dedicated care team. You will deliver planned, patient-centered nursing care to residents in alignment with provincial standards, within a supportive multidisciplinary environment.

As part of our team, you will document assessments and care plans, teach and supervise unregulated care providers, and communicate clearly with residents, families, and colleagues to ensure quality care.

Qualifications

• Current registration as an RPN in the applicable province.
• Experience with seniors and unregulated care providers preferred.
• Gerontology, rehabilitation, or long-term care experience is an asset.
• Strong teamwork, customer service, and communication skills.
• Excellent organizational and time-management skills.

Responsibilities

• Provide planned, patient-oriented nursing care per provincial standards.
• Collaborate with multi-disciplinary team and maintain communication.
• Implement and evaluate care plans; adjust as needed.
• Complete resident assessments, care plans, and maintain documentation.
• Teach and direct unregulated care providers with coaching.

How YOU make a difference

• RPNs are responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.
• RPNs are an integral part of our multi-disciplinary team who build trusted relationships with our residents, families and teams with continuous ongoing and effective communication.
• RPNs implement and evaluate the effectiveness of the care plans and make changes as required.
• RPNs complete and ensure resident’s clinical care assessments, care plans and documentation are accurate, up to date and communicate changes as required.
• RPNs teach, delegate and direct unregulated care providers, providing them with coaching & mentorship.

What YOU bring

• Current registration as a Registered Practical Nurse in the applicable province
• Demonstrated sound clinical knowledge
• Experience working with seniors and with unregulated care providers preferred
• Education and experience in gerontology, rehabilitation, community/complex/long term care an asset
• Strong drive, respect and an understanding of the emotional and physical demands of the role
• Focus on teamwork and customer service with the ability to build trusted relationships
• Strong written and oral communication skills and critical thinking skills
• Demonstrated organizational and time management skills
• Computer proficiency and experience with Microsoft office applications
• Current Basic Cardiac Life Support and First Aid certification required
• Current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
