Job summary

First Light is seeking an Inclusion Case Manager to join a 10-unit transitional housing program serving Indigenous communities, including Two-Spirit and gender-diverse members. You’ll provide individualized support, navigate systems, and coordinate referrals with resident consent.

Your role emphasizes culturally grounded, trauma-informed, harm-reduction approaches and collaboration with a residential team to promote safety, housing stability, and independence.

Qualifications

• Diploma or degree in Social Work, Indigenous Studies, or related field.
• 2–3 years of relevant case management experience.
• Understanding of Indigenous cultures and colonization impacts.
• Understanding of intersections of Indigeneity, gender diversity, housing instability, trauma and violence.
• Strong communication, documentation, advocacy, and conflict-resolution skills.
• Valid driver’s license with reliable vehicle and clean driving record.
• Satisfactory Record of Conduct and Vulnerable Sector Check.

Responsibilities

• Develop and review individualized, resident-led support plans reflecting goals, culture, safety needs.
• Provide one-to-one support and practical assistance to residents in First Light’s housing program.
• Support housing stability, budgeting, food security, appointments, daily living skills, self-advocacy, and longer-term housing prep.
• Help residents navigate government, health, income-support, legal, housing, and community systems.
• Coordinate referrals and service planning with internal and external partners with resident consent.
• Monitor progress and communicate updates to the Team Lead of Residential Services.
• Contribute to a welcoming, inclusive, culturally safe environment grounded in dignity and harm reduction.
• Support orientation, community agreements, conflict resolution, and shared living practices.
• Respond to urgent or crisis situations per First Light policies.
• Document incidents, safety concerns, and outcomes with required timelines.
• Maintain service and caseload coverage during staff absences.
• Connect residents with Elders, Knowledge Keepers, and First Light programming.
• Develop and deliver culturally relevant, gender-affirming activities.
• Foster empowerment and intergenerational connection; advocate with systems as needed.
• Participate in supervision, team meetings, training, and professional development.
• Collaborate to ensure coordinated service delivery and maintain confidentiality.
