Job summary

Dilico Anishinabek Family Care seeks a Treatment Services Case Manager to provide culturally responsive case management for adults through an Indigenous trauma-informed framework. The role supports mental health, emotional, behavioural, and psychosocial challenges via counselling, advocacy, and resource access.

Responsibilities include intake coordination, waitlist management, and service navigation, with collaboration among families, communities, and multidisciplinary teams to promote client

Qualifications

  • Minimum HBSW or related degree and registration/eligibility with a regulatory body preferred.
  • Experience in intake, screening, assessment, and risk identification.
  • Experience in short-term counselling and walk-in/brief interventions.
  • Criminal records search satisfactory and ability to travel within Thunder Bay District.

Responsibilities

  • Provide culturally responsive case management to adults in an Indigenous trauma-informed framework.
  • Coordinate waitlist, intake, and service navigation for counselling.
  • Develop individualized support plans addressing identified goals and priorities.
  • Document clinical information accurately and participate in case reviews.
