Job summary

Friends of Ruby in Toronto is seeking a Case Manager to provide direct case management and community-based support to high-risk 2SLGBTQIA+ youth, focusing on housing, mental health, and social services through a trauma-informed, anti-oppressive approach.

You will conduct assessments, develop care plans, coordinate services, and lead life-skills workshops while maintaining records and collaborating with internal teams. Hybrid onsite schedule, evenings/weekends may be required.

Qualifications

  • Post-secondary degree Social Work, Psychology, or a related field (or equivalent social services experience).
  • Experience in specialized case management within mental health, supportive housing, and crisis intervention environments.
  • Experience working in not-for-profit organizations, 2SLGBTQIA+ community settings, social justice environments, and/or housing and shelter programs.

Responsibilities

  • Conduct comprehensive assessments and develop individualized care plans to meet youth needs.
  • Provide 1:1 case management and crisis intervention using trauma-informed and harm reduction approaches.
  • Facilitate life-skills workshops and group sessions to promote independence and resilience.
  • Coordinate internal and external services, advocating for youth access to housing, health care, and social supports.
  • Collaborate internal teams to support integrated care planning.
  • Maintain accurate clinical records and prepare reports in a timely manner in compliance with legal and ethical standards.
  • Engage directly with participants in community settings to assess needs, provide support, and promote access to services.
  • Engage in community outreach and build partnerships to strengthen service delivery and resource access.
  • Support implementation of public health protocols and organizational policies to ensure safety and compliance.
  • Participate actively in clinical supervision, team meetings, to contribute to team learning, uphold service quality, and respond to evolving operational and crisis needs.

The Case Manager provides direct case management and community-based support to high-risk 2SLGBTQIA+ youth, ensuring access to housing, mental health, and social services through a trauma-informed, anti-oppressive approach.

As the Case Manager you will be responsible for:

Required Credentials
  • Post-secondary degree Social Work, Psychology, or a related field (or equivalent social services experience).
  • Experience in specialized case management within mental health, supportive housing, and crisis intervention environments.
  • Experience working in not-for-profit organizations, 2SLGBTQIA+ community settings, social justice environments, and/or housing and shelter programs.
  • Familiar with case management and housing systems - SMIS, TREAT, Greenspace, RENTCafe, and My Access to Housing Toronto considered an asset.
What We Need
  • Must be legally authorized to work in Canada.
  • Must provide a satisfactory Vulnerable Sector Police Check.
  • Must reside in Ontario.
  • Must be work onsite in downtown Toronto on a hybrid schedule.
  • Must have flexibility to work evenings and weekends occasionally as required.
  • Travel to various community-based settings as needed
