Job summary

Across Boundaries is a mental health center located at 51 Clarkson Ave., Toronto, ON. We hire a permanent full-time Case Manager to support individuals with serious mental illness, addictions, and criminal justice involvement, serving racialized communities across the Greater Toronto Area.

The role reports to the Program Manager and involves a typical 20–25 client caseload with varied hours, occasional Sundays, and on-site work. Relevant experience and a commitment to anti-racism are essential.

Qualifications

• Degree in Social Work from a recognized university or equivalent.
• At least two years of experience as a case manager.
• Knowledge of the Criminal Justice System and Ontario Mental Health Act.

Responsibilities

• Provides intensive case management to individuals experiencing serious mental illness and dealing with possible addictions issues.
• Linkage and coordination of services with partner community agencies.
• Accurate record keeping and timely documentation.
• Establish recovery goals in areas such as life skills, independent living, crisis prevention, budgeting, community integration, symptom management and employment.
• Provide interventions such as advocacy, referral to resources, counselling, crisis intervention, and psychoeducation.
