Your Support Services Network (YSSN) in Aurora, Ontario is seeking a Mental Health Case Manager for a full-time contract through September 2027. The role provides intensive case management and advocacy to adults with mental health needs, homelessness, or related challenges, with daily travel in York Region.

One Full-Time Contract position to September 2027 (with the possibility of extension or becoming permanent)
This position will provide intensive case management and advocacy services to individual’s aged sixteen and over who are experiencing mental health, dual diagnosis and/or addictions and who are vulnerably housed or homeless. Through a collaborative approach, this position will facilitate recovery and assist service users who are experiencing significant difficulties in daily functioning to improve their quality of life. Tasks will include direct service interventions, clinical support and community development. Daily travel within York region will be required.
* The successful applicant will be required to provide a police clearance that meets agency standards
* A valid G Drivers License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation.
