Job summary

Durham Community Health Centre (DCHC) is seeking a Case Manager, Welcoming Streets to provide outreach and individualized case management using the Housing First approach. The role targets unsheltered clients and connects them with housing supports, health and social services.

The successful candidate will work within an interdisciplinary team, travel within Durham Region, and adapt to client needs in a dynamic community setting. This is a one-year contract, with evenings/weekends as required.

Qualifications

• Formal 2-year post-secondary program in social services.
• Experience with street-involved populations.
• Ability to work with marginalized and culturally diverse populations.
• Crisis intervention skills.
• Knowledge of harm reduction, health education, and peer support models.
• Strong communication, advocacy and negotiation skills.
• Must have a valid driver’s license and access to a vehicle.
• COVID-19 vaccination is mandatory for this position.

Responsibilities

• Deliver outreach services, including support, education, and referrals.
• Advocate for housing, food, and financial assistance resources.
• Develop, implement and evaluate the program and client service plans.
• Identify and connect at-risk individuals with housing supports.
• Build partnerships with local service providers for coordinated care.
• Represent the program in external meetings and committees.
