Job summary

CMHA Peel Dufferin Branch seeks a Relief Outreach Worker to support our outreach team in Brampton. This on-site role uses Housing First principles to connect unsheltered or at-risk individuals with housing and wraparound supports.

You will build relationships, conduct intake assessments, collaborate with Region of Peel Housing Stability Workers, and maintain records while promoting recovery and client empowerment.

Qualifications

  • Post-secondary degree or diploma in relevant discipline (e.g. BSW, MSW, OT, RN)
  • Minimum of 3 years of experience supporting those experiencing homelessness in a community setting
  • Experience in outreach with demonstrated understanding of Outreach practices is an asset
  • Previous case management experience is preferred
  • A personal experience with mental health, addiction and/or homelessness and recovery is an asset
  • Training in Housing First, Harm Reduction, Motivational Interviewing, Assertive Engagement, Indigenous Cultural Safety, Non-violent Crisis Intervention, ASIST, CPR and First Aid are assets
  • Strong interpersonal skills with ability to communicate across diverse cultures and intersectionality; anti-racist, anti-oppressive approach
  • Valid Canadian driver’s license and access to insured vehicle; Vulnerable Sector Screening required

Responsibilities

  • Build client capacity, empowerment and self-reliance
  • Develop relationships with prospective clients based on trust and respect
  • Assist in securing affordable housing for those at risk of homelessness in Peel
  • Support housing transitions using a Housing First approach and document support plans
  • Complete initial and discharge assessments and related documentation
  • Conduct case conferencing with the client’s support system
  • Maintain relationships with referral sources and service providers
  • Make weekly community/home visits to offer support and connect to resources
  • Advocate with participants (e.g., landlord relations, tenancy boards)
  • Collaborate with Peel Housing Stability Workers to teach life skills for tenancy
  • Maintain records, statistics and outcomes per CMHA/Peel policy
  • Provide input for policy, service improvement and program evaluation
  • Support Street Outreach and Street Helpline operations
