Canadian Mental Health Association (CMHA) York Region & South Simcoe is hiring a Case Manager to operate within a Housing First model. The role requires collaboration with clients to secure housing, promote independence, and support community integration in York Region and South Simcoe, including Aurora office duties.

The position is contract, full-time (35 hours/week) with occasional evenings/weekends, travel within the region, and a closing date of 2026-11-04.

Qualificações

• Degree or diploma with relevant experience in a community-based mental health setting.
• Current registration with a regulatory body under applicable acts (e.g., OC SWSSW).
• Bilingual or second language is an asset.
• Knowledge of Ontario mental health legislation and related acts.

Responsabilidades

• Adheres to Intensive Case Management Standards of the Ontario Ministry of Health and Long-Term Care.
• Maintains a client caseload.
• Initiates referrals to CMHA programs or external agencies as required.
• Develops collaborative partnerships with clients using recovery/empowerment principles.
• Communicates effectively and maintains confidentiality.
