Job summary

Canadian Mental Health Association, York is seeking a Case Manager to operate within a Housing First model. The role focuses on housing-focused support, individualized plans, and collaboration with clients to promote recovery and independence.

You will work from the Aurora office, travel within York Region and South Simcoe for meetings, and may assist across programs such as Flexible Support or ACTT as needed.

Qualifications

• Degree or diploma with relevant mental health experience in the homelessness sector preferred.
• Current registration with the regulatory body; not in qualifying stages.
• Ability to work in York Region and South Simcoe communities.

Responsibilities

• Adheres to Intensive Case Management Services Standards.
• Maintains a client caseload per standards.
• Initiates referrals to CMHA programs or external agencies.
• Collaborates with clients to set goals and promote independence.
• Ensures confidentiality and ethical practice.
