Job summary

Canadian Mental Health Association Huron Perth Addiction and Mental Health is seeking a Case Manager for the Heart to Home Bridge Program. The role focuses on housing stability for individuals facing homelessness within a Housing First framework.

You will provide intensive case management, including intake, triage, and connections to services, while promoting dignity and self-efficacy. Responsibilities include documentation, collaboration with care providers, and flexible hours.

Qualifications

• Degree or diploma in health or social services; registration with a professional college preferred.
• Minimum of three (3) years’ work experience in a social service field.
• Knowledge of SPDAT and other standardized assessments is an asset.
• Strong understanding of homelessness, mental health, addictions, legal/justice issues, victimization and trauma.
• Ability to work independently and within a multi-disciplinary team.

Responsibilities

• Advocate for participants with external service providers and maintain collaborative relationships.
• Maintain participant confidentiality according to agency policies.
• Represent the agency through community awareness, education, and outreach activities.
• Attend team and planning meetings and participate in supervision and professional development.
• Maintain an active caseload of approximately 10–15 participants and monitor progress toward housing stability.
