Cota Health is seeking a Case Manager, Addictions Specialist to provide Recovery Oriented services to clients in encampments in East Toronto. The role supports housing, health access, and education within an Assertive Housing First framework.

You will conduct assessments, develop safety and recovery plans, and coordinate community resources, with a focus on harm reduction and client-centred care in a dynamic outreach setting.

Formación

• A combination of post-secondary degree/diploma and 2-3 years direct experience, relevant to the target population.
• Or a combination of BSW, MSW or other regulated health care professional designation and 1 year direct experience.
• Essential qualifications include an Addictions Certificate or MH & Addictions Diploma/Degree.

Responsabilidades

• Review referral information and determine eligibility for the program.
• Coordinate screening interviews with applicants and referral sources.
• Develop a collaborative relationship and personalized safety and recovery plans.
• Provide direct practical support including shelter, income, ID and clothing.
• Coordinate group activities and educational sessions for clients.
• Document client contacts and enter data per guidelines.
• Adhere to policies, safety requirements and confidentiality.
• Travel to encampments and multiple community settings to provide supports.
