Qualifikationen

• Educational background in social services with a mental health and harm reduction focus.
• Minimum 2-year experience working with people living with serious mental health/substance use issues and related challenges.
• Knowledge of harm reduction principles and overdose prevention training.
• Experience with data management and reporting in social service settings.

Aufgaben

• Assist clients in developing case management plans and conduct regular check-ins.
• Assist clients with housing search, landlord coordination, and referrals for furniture.
• Coordinate care with service providers and assist with medical and health care access.
• Prepare monthly and quarterly statistical reports and maintain client records per standards.
• Participate in meetings and supervise sessions to ensure service quality.
• Lead crisis interventions and maintain professional boundaries with clients and partners.
