Job summary

BayCare Health System in Tampa, FL is seeking a Part-time Hospital Care Coordinator focused on RN case management. You will provide linkage, monitoring and advocacy to maximize patient functioning, coordinate discharge planning and ensure resources support quality outcomes.

The role requires RN licensure, 2+ years in acute care, and experience in case management. ACM/CCM are preferred. Associates of Nursing is required; Bachelor’s is preferred.

Qualifications

• RN (Registered Nurse) required.
• ACM (Case Management) preferred.
• CCM (Case Manager) preferred.
• Associates of Nursing required.
• Bachelor of Nursing preferred.
• 2 years in Acute Care required.
• 2 years Registered Nurse required.

Responsibilities

• Provides linkage, monitoring, planning and advocacy for our population to achieve and maintain maximum functioning.
• Possesses a clear understanding of the available resources to promote quality cost effective outcomes.
• Effectively maintains and manages assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served.
• Coordinates discharge planning efforts through assessment, referral, and interdisciplinary planning.
• Ability to adjust to changing priorities in a fast‑paced environment.

Position Details

• Location: St Josephs Main and Childrens, Tampa FL
• Status: Parttime
• Schedule: 8:00 AM – 4:30 PM
• Weekend Requirement: Every Other
• On Call: None
