ParaMed Home Health Care is seeking a Clinical Planner to onboard new patients requiring Personal Support Services in an assigned district. You will assess needs, implement care plans and ensure safe environments for care delivery.

The role emphasizes coordination of scheduling, patient and family communication, and maintaining accurate records while upholding privacy and cultural sensitivity within a leading national home health provider.

Responsibilities

• Perform initial assessment of care needs for new patients and establish individualized care plans.
• Conduct initial safety assessment of patient living environment for safe work conditions.
• Provide initial personal care services for new patients.
• Advise on scheduling parameters for ongoing visits based on clinical needs.
• Inform patients and families about ParaMed policies, scheduling practices and expected care times.
• Maintain current and accurate patient records.
• Respect cultural needs, dignity, privacy, and confidentiality of patients and staff.
• Stay updated on changes in practice that affect scope of practice.
