Job summary

Ontario Health West is seeking a Lead, Capacity, Access and Flow to drive regional and local collaboration, improve patient flow and transitions, and reduce avoidable bed days. You will partner with hospitals and health providers, analyze health-system data, and lead complex improvement initiatives across the continuum of care.

The role requires strong project management, change leadership, and the ability to translate data into practical strategies that benefit patients and the broader health

Qualifications

• 5+ years in a multi-stakeholder health environment.
• Strong planning, analysis and problem-solving abilities.
• Experience leading complex health-care initiatives and change management.

Responsibilities

• Support regional and local partners to implement best practices improving access and patient flow.
• Use health-system data to identify gaps and propose practical solutions.
• Build trusted partnerships with hospitals, providers, and stakeholders to drive change.
• Lead initiatives to improve hospital patient flow and reduce unnecessary bed days and ALC days.
• Help establish performance measures and translate findings into action.
• Advance quality-improvement practices reflecting patient and caregiver perspectives.
• Ensure adherence to health-service accountability agreements and foster a respectful workplace.
• Perform other duties as assigned.
