Job summary

Fraser Health is seeking an Access & Flow Leader to coordinate patient placement and bed utilization across MH&SUS sites, mainly on weekends. You will lead Patient Care Coordinators and Charge Nurses, manage capacity, and drive safe patient flow across FH sites.

Ideal candidates are Registered Nurses with leadership experience, strong decision-making, and collaboration skills across disciplines. Travel between FH MH&SUS sites is required to ensure timely access to care.

Qualifications

• Bachelor's Degree in Nursing and current practicing registration with the BC College of Nurses and Midwives (BCCNM).
• Five (5) years' recent, relevant acute care experience with mental health and substance use services, including at least two (2) years in supervisory/administrative capacity.
• Valid BC Driver's License and access to a personal vehicle for business-related purposes.

Responsibilities

• Directs Patient Care Coordinators (PCCs) and/or Charge Nurses and coordinates appropriate patient placement and/or discharge needs.
• Facilitates timely patient placement at designated FH acute sites; assesses capacity and expedites actions; reports to AOC managers.
• Accountable for optimal use of patient care resources; coordinates staffing and overtime per needs.
• Consults with PTN and community resources to identify appropriate care and transfers.
• Oversees staff education and advises on bed utilization policies and procedures.
