Extendicare is seeking a Registered Practical Nurse to provide patient‑oriented, goal‑directed nursing care in a senior care environment in Ontario. You will collaborate with the interprofessional team to develop and implement care plans, conduct assessments, and ensure accurate documentation and communication with the care team.

Qualifikationen

• Current registration as a Registered Practical Nurse in Ontario.
• Experience in a seniors’ environment preferred.
• Strong written and oral communication and critical thinking skills.

• Work with interprofessional team to provide person-centered nursing care.
• Complete resident assessments, care/service plans, and documentation.
• Implement, evaluate and update care plans as needed.
• Assist with scheduling care conferences and transfers.
• Provide medication administration and education per policies.
• Coordinate with care team to ensure timely service delivery.
