Job summary

MedMatch Recruitment is seeking a compassionate Nurse Practitioner in a long-term care setting in Ontario. You will provide primary care to residents, collaborate with an interdisciplinary team, and lead care plans to promote health and reduce hospital transfers.

Qualifications

• Must hold NP designation with the College of Nurses of Ontario (CNO).
• Experience in geriatrics/primary care/long-term care preferred.
• Current CPR certification.
• Vulnerable sector check clearly completed.
• Familiarity with Ontario healthcare regulations and EMRs.

Responsibilities

• Provide advanced nursing care, including assessment, diagnosis, and treatment planning.
• Manage chronic conditions such as diabetes, hypertension, COPD, and dementia.
• Perform routine health assessments and wellness checks.
• Prescribe medications and treatments in accordance with NP scope of practice in Ontario.
• Collaborate with physicians, nurses, PSWs, pharmacists, and other healthcare professionals.
• Lead care conferences and communicate with families as needed.
• Respond to urgent medical concerns and coordinate acute care when necessary.
• Promote resident independence and preventative care strategies.
• Maintain accurate and timely medical documentation in compliance with regulatory standards.
• Support staff education and development in areas of clinical best practices.
