Job summary

Partners Community Health’s Integrated Seniors Hub in Mississauga is seeking a Part-Time Nurse Practitioner to provide direct comprehensive care to clients with specialized needs. You will work to your full scope of practice within an interdisciplinary team to support residents’ health and quality of life.

The role emphasizes geriatric care, collaboration with physicians, nurses, PSWs, and SDMs, and evidence-informed decision making.

Qualifications

• Registered Nurse in Ontario Extended Class
• Master's degree preferred for advanced practice
• Ontario NP certification and active practice required

Responsibilities

• Provide comprehensive nurse practitioner care to seniors with complex needs.
• Collaborate with physicians, nurses, PSWs and SDMs within an interprofessional team.
• Prescribe pharmacotherapy based on patient health history and guidelines.
• Document interventions and outcomes in timely, accurate fashion.
