Job summary

AgeCare Ltd. is recruiting a Nurse Practitioner for Generations Toronto, a not-for-profit intergenerational campus in Toronto. The NP will work with the MD, DOC and interdisciplinary team to provide resident care, diagnose and manage acute and chronic conditions, and support care planning and family SDMs.

The role emphasizes clinical leadership, education, and participation in care conferences to enhance quality and safety for residents in a long-term care setting.

Qualifications

• Minimum one year clinical experience in geriatric, rehabilitation, or long-term care.
• Leadership knowledge/experience is an asset.
• Certifications in gerontology, mental health, palliative care are assets.
• Current registration with the College of Nurses of Ontario as an RN in the Extended Class (PHC or Adult) and entitled to practice.

Responsibilities

• Collaborate with the MD, DOC and interdisciplinary team in care delivery.
• Assess, diagnose, and evaluate acute and chronic illness and other changes in resident's conditions, prescribe treatments, diagnostics and follow up care.
• Support and advocate for residents' best interests while engaging with the resident/SDM regarding care.
• Participate in resident care conferences.
• Provide clinical leadership to the LTC team centered around resident care needs including move‑in/out, discharge planning, palliative and end-of-life care, pain management, dementia care and responsive behavior management.
• Identify, analyze, and interpret trends in resident care to determine priorities for educational programming for team members, residents, and families.
