Job summary

AgeCare is seeking a Registered Practical Nurse (RPN) to provide professional nursing care to residents in our long-term care communities across Ontario. You will work under the direction of the Registered Nurse and support the care team to ensure residents receive high-quality, compassionate care.

You will collaborate with physicians and health care colleagues, mentor Personal Support Workers, participate in shift handovers, and help shape care procedures in the unit.

Qualifications

• Geriatric, rehabilitation or long-term care nursing background including activation and personnel administration.
• Knowledge of relevant acts and regulations affecting long-term care in Canada and Ontario.
• Current Certificate of Competence from the College of Nurses of Ontario in good standing.

Responsibilities

• In collaboration with the RN, implements AgeCare policies on the unit and inputs into procedure revisions.
• Liaises with physicians and the health care team; notifies physician of significant resident changes in RN's absence.
• Works with the care team to meet residents' care needs and provide direction to Personal Support Workers.
• Participates in shift reports to inform the team of changes in resident status.
