Job summary

AgeCare Ltd. is seeking a Registered Practical Nurse (RPN) to provide professional nursing care to residents in Ontario's long-term care communities, in collaboration with the Registered Nurse and the care team.

The role requires adherence to College of Nurses standards, supervision of Personal Support Workers, and liaison with physicians, ensuring high-quality care in a supportive, patient-centered environment.

Qualifications

• Experience in geriatric, rehabilitation or long-term care nursing.
• Familiarity with relevant Canadian health acts and regulations.
• Current Certificate of Competence from the College of Nurses of Ontario.

Responsibilities

• In collaboration with the Registered Nurse, implements AgeCare policies and procedures on the unit.
• Provides input into the revision of procedures within the residence and the unit.
• Liaises with physicians and other health care team members including external partners.
• In the absence of the Registered Nurse, notifies the physician of significant changes in resident status.
• Works with health care team to ensure residents' care needs are met.
• Provides direction to Personal Support Workers on the care team.
• Participates in shift reports ensuring all team members are informed of changes in resident status.
