Qualifications

• Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
• Current College of Nurses of Ontario (CNO) registration in good standing.
• A valid driver’s license and vehicle as regular travel is required.

Responsibilities

• Develop and maintain an effective Care Plan respecting Community Health Services.
• Provide exceptional nursing services in multiple settings.
• Communicate with patients and interdisciplinary care team.
