Job summary

Six Nations of the Grand River seeks a Case Manager to provide comprehensive Ontario Works case management in a First Nations community. You will assess needs, formulate personalized service plans, and connect clients with resources to foster independence.

Responsibilities include documentation in the information system, referrals to community programs, and regular client meetings. Travel within the community and to related agencies is required.

Qualifications

• University or College Diploma in Social Work/Human Services or related field with one year of experience in Social Assistance or related Human Service setting.
• Ontario Secondary School Diploma or Equivalent with at least three years of experience in a Social Assistance or related Human Service setting
• Knowledge of the Ontario Works Act, Regulations and Policy Directives
• Valid driver's license and access to a reliable vehicle
• Successful completion of a criminal background check

Responsibilities

• Conducts detailed assessments of employability needs and barriers for clients, spouses, and adult dependents
• Prepares and makes referrals to community programs and external agencies
• Develops phased participation plans addressing employment barriers and referrals to SATF projects
• Monitors continued eligibility for financial assistance and client participation
• Schedules regular meetings for supportive counseling, skills training and reviews
• Maintains confidential client records and prepares reports
