Job summary

Six Nations of the Grand River invites applications for the First Link Care Navigator contract role. Based out of Six Nations, you will coordinate supports for persons living with dementia and their care partners, serving as the primary contact after diagnosis and driving care planning across health and community services.

You will lead information sharing, problem solving, and liaison with physicians, hospitals, and community providers to enhance home-based and community care outcomes.

Qualifications

• Bachelor degree in social work, gerontology, nursing or related health care discipline.
• Registered health professional designation; master’s level education preferred.
• 3 to 5 years client service experience in health and/or social services.

Responsibilities

• Coordinate and integrate supports for persons with dementia and their care partners.
• Serve as the main go-to contact for families after diagnosis, supporting self-management goals.
• Facilitate care conferences and coordinate across providers and sectors.
• Maintain client records with privacy and professional standards.
