Our Complex Care Support Team in Grande Prairie, supporting both community and clinic patients, is currently seeking a Nurse Practitioner (NP) with expertise in geriatric psychiatry and dementia to join our team.
In this role, you’ll provide comprehensive care to adult patients across various care settings while closely collaborating with a multidisciplinary team. You’ll utilize best practices to optimize patient care and support individuals, families, and primary care providers in the North Zone.
As a leader and resource in your field, you’ll mentor fellow team members, lead research initiatives, maintain effective relationships with stakeholders, and engage with clients and families as partners in care. This position involves both face-to-face patient interactions and virtual consultations with clients and the interdisciplinary team. In the future, participation in an on-call rotation for 24/7 virtual NP consultation coverage throughout the North Zone may be required.

As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to meet patients' health needs. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership, Health Systems, Education, and Research to improve care quality, promote appropriate health service use, and inform healthcare systems.

Required Qualifications:
Successful completion of an approved Nurse Practitioner program with a minimum of a master’s degree. Registration with CRNA on the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP).
Additional Required Qualifications:
Minimum of 5 years of clinical experience as a Nurse Practitioner. At least 1 year of clinical experience managing complex clients with dementia within the last 3 years. Minimum 1 year of Seniors Health experience. Advanced clinical and assessment skills. Connect Care trained. Proficient with MS Office Suite and TEAMS. Proven leadership experience. Valid driver’s license and access to a vehicle for work-related travel. Ability to work independently in various environments and physically capable of carrying approximately 25-30 lbs.
Preferred Qualifications:
Experience in at least 2 care sectors, including Seniors Health. CNA Certification in Geriatrics or willingness to obtain. Case Management Education. Experience in program planning, adult education, interdisciplinary practice, and research activities. Excellent interpersonal and communication skills. Ability to work independently and adapt to changing clinical situations. Collaborative team player, including with patients and families.
