Job summary

Qualifications

• Health Care Aide Certificate from a recognized program.

Responsibilities

• Assist clients with activities of daily living per care plan.
• Document care and report changes to team leader.
• Take and record vital signs as directed.
• Provide hygiene, grooming, mobility and feeding assistance.

• Support admission, transfers, and discharge processes.
