Responsibilities

• Assist clients in activities of daily living, promoting self-respect and independence.
• Follow individualized care plans and report client changes to the team leader.
• Document care according to policy and record observations.
• Provide personal hygiene and basic care per care plan.
• Maintain safety and cleanliness in the client environment.
• Support patient observation, vital signs, and reporting as directed.
• Participate in admissions, transfers, and discharge processes.
• Contribute to infection control and hand hygiene practices.
