Job summary

Southwest Ontario Aboriginal Health Access Centre (SOAHAC) in Cambridge, ON seeks an Indigenous Patient Navigator to support Aboriginal clients and families as they access hospital and community health services. You will coordinate care planning, provide culturally appropriate navigation, and connect clients with supports to transition safely home.

The role emphasizes trauma-informed, client-centered care and collaboration with an interdisciplinary team; travel is a requirement.

Qualifications

• Three years of FNIM-focused work in hospital or community settings.
• Knowledge of case management and navigation models.
• Knowledge of mainstream health services and FNIM programs in Grey Bruce area.
• Indigenous cultural competency and ability to translate to health care providers.
• First Aid/CPR certification required; license in good standing.

Responsibilities

• Assist Indigenous clients and families in navigating the health care system with compassion.
• Provide clear information about diagnoses, treatment options and care plans.
• Coordinate appointments, referrals and follow-ups for timely care.
• Connect clients with community programs, financial supports and culturally relevant resources.
• Advocate for client needs and reduce barriers to care.
• Foster communication among clients, providers and care teams.
• Promote culturally safe, trauma-informed, client-centered care practices.
• Maintain records of client interactions and track outcomes for quality improvement.
