Job summary

Partners Community Health is launching an integrated Seniors Hub in Mississauga to provide digitally enabled navigation and coordinated care planning for seniors. The Navigator will support plan development, coordinate referrals, and collaborate with MOHT and local partners to promote access to services.

Qualifications

• Membership in good standing with Ontario regulatory bodies such as nursing, OT, physiotherapy, social work, or speech-language pathology is required/strongly preferred.
• 2+ years of experience in community health or related field is expected.
• Knowledge of the health care delivery system and community resources.
• Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
• Experience with seniors in LTC is an asset and appreciated.

Responsibilities

• Support the development and implementation of access and navigation plans for seniors services in alignment with strategic partners and regulatory requirements.
• Enhance workflows and communication between PCH and care providers to promote seamless care.
• Create opportunities for community members to connect with PCH partners and raise awareness of services.
• Refer clients to clinical staff and long-term care professionals as needed.
• Build and maintain relationships with network partners and relevant services.
• Contribute to a safe, welcoming care atmosphere and person-centered education about PCH programs.
