Job summary

Environment, Conservation and Parks seeks registered nurses to join correctional facilities across Ontario. The roles include permanent and casual on-call hours, with a broad scope from emergency to chronic care in an unconventional setting.

You will work as part of an inter-professional health care team to deliver patient-centered care and ensure continuity of care. Requirements include current RN with the College of Nurses of Ontario in good standing.

Qualifications

• Must be a Registered Nurse in good standing with the College of Nurses of Ontario.

Responsibilities

• Perform comprehensive health assessments and develop patient-specific care plans.
• Advocate for patient participation in care plans and coordinate with inter-professional team.
• Provide emergency, acute care, crisis intervention, health teaching and counselling as needed.
• Collaborate with community care providers to obtain health histories and facilitate transitional care.
