Job summary

Ontario Public Service (OPS) is seeking a Nurse Practitioner to provide patient-centred primary care at the Algoma Treatment and Remand Centre. You will work within an inter-professional team delivering health care in a correctional setting.

The role focuses on assessment, diagnosis, treatment, and health promotion, while collaborating with staff to optimize care plans and patient outcomes in a rotating 7-day service.

Qualifications

• Registered Nurse Extended Class (Nurse Practitioner) in good standing with the College of Nurses of Ontario.
• Nurse Practitioner specialty certificate in primary health care or adult care.
• Provide patient-centred primary care in a correctional setting.
• Assess, diagnose, and prescribe treatment with focus on health promotion and disease prevention.
• Collaborate with an inter-professional team and involve offenders in their plan of care.
• Lead and participate in quality improvement and coordination of primary health services.
• Knowledge of illness, pharmaceuticals, diagnostics, and mental health/addiction treatment.
• Communicate clearly; document care; tailor communication for diverse populations.
• Hours are 36.25 per week with rotating shifts in a 7-day service.

Responsibilities

• Provide primary health care services in a correctional facility as part of a health care team.
• Assess, diagnose, prescribe and monitor treatment; promote health and disease prevention.
• Collaborate with colleagues to develop patient plans of care and involve offenders in decision making.
• Respond to emergencies and apply appropriate interventions.
• Lead and participate in quality improvement and service coordination.
