Job summary

AgeCare is seeking a Registered Practical Nurse (RPN) to provide professional nursing care to residents in Ontario communities. The role collaborates with the Registered Nurse and directs Personal Support Workers to ensure care aligns with standards and procedures.

The position involves liaising with physicians and the health care team, communicating changes during shifts, and contributing to a compassionate, diverse team focused on meaningful resident outcomes.

Qualifications

• Knowledge of geriatric, rehabilitation or long-term care nursing.
• Knowledge of the Long-Term Care Act, Program Standards for Long Term Care, the Health Disciplines Act, OHSA, Pharmacy Act, Food and Drugs Act (Canada), Narcotic Control Act (Canada), Coroners Act, College of Nurses of Ontario Standards of Practice.
• Current Certificate of Competence from the College of Nurses of Ontario and a member in good standing.

Responsibilities

• Implements AgeCare policies and procedures on the unit in collaboration with the Registered Nurse.
• Liaises with physicians and other members of the health care team including external health care partners; notifies physician of significant changes when the RN is unavailable.
• Works with health care team to ensure residents' care needs are met.
• Provides direction to Personal Support Workers on the care team.
• Participates in shift reports to inform team of changes in resident status.
