Extendicare seeks a part-time Registered Practical Nurse (RPN) to enhance nursing practice quality and patient-centered care across our Canadian long-term care homes. You will perform assessments, develop and update care plans, and document actions in accordance with regulatory standards and Extendicare policies.

You will work within an inter-professional team, coordinate care with physicians and staff, and respond to clinical changes while ensuring safe medication administration and patient

Mansioni

• Work as a member of the inter-professional team applying Extendicare's Professional Practice Model and person-centered care.
• Complete and ensure residents' clinical care assessments, care plans, and documentation are accurate and up to date.
• Implement and evaluate care/service plans and communicate changes as required.
• Assist with scheduling resident care conferences and participate as applicable.
• Collaborate with the Director of Care to facilitate resident move-ins/outs and transfers.
• Provide direct resident care and administer medications/treatments in line with policies and regulatory requirements.
• Document care and ensure safe handling of drugs and narcotics per policy.
