Site: Rumsey Centre, Toronto Rehab

Reports to: Program Services Manager
Scale/Grade: D0:04
Hourly rate: $29.66 - $37.07 hour
Hours: 37.5 hours per week weekdays
Status: Temporary Full-time, 12-month contract with opportunity for renewal for an additional 12 months
Closing Date: January 31, 2025

JOB SUMMARY:

The Community Support Worker (CSW) is a member of the LIFEspan interdisciplinary team in the Brain Program at Toronto Rehab. This role supports the implementation of the Future Cares Initiative, focusing on supporting clients and caregivers with longer-term planning and successful community transition and integration. The CSW’s focus is on supporting the complex needs of clients and their loved ones in creating a long-term plan that includes, but is not limited to, subject areas such as legal supports, housing, social/recreational, meaningful activities, employment/volunteer opportunities, and physical and mental health care. The successful candidate will accomplish this by providing one-to-one instrumental supports to LIFEspan clients and their loved ones, advocating for clients and families around community resources, as well as facilitating both psycho-social groups and psycho-educational workshops within these subject areas, as deemed appropriate. The CSW participates in program planning and quality improvement activities; engages in program and hospital professional development; and collaborates with inter-professional teams as well as community-based and private service providers.

Key Responsibilities

  • Provide information and guidance on available resources and services tailored to each client's goals and needs.
  • Support clients in developing their own life plans or guides based on what's important to them.
  • Help clients navigate the Coriat Cares Fund application process.

Project Assistance:

  • Collaborate with the Future Cares Initiative team to implement project initiatives.
  • Maintain up-to-date client records and documentation related to future care planning and fund applications.
  • Assist in monitoring and evaluating the effectiveness of support services provided.

Resource Development:

  • Develop informational resources for LIFEspan clients and their loved ones.
  • Ensure all resources are accessible, user-friendly, and meet the diverse needs of our client population.

Workshop Coordination:

  • Assist in the planning, organization, and implementation of workshops.
  • Coordinate logistics, materials preparation, and participant communication.
  • Support outreach efforts to encourage LIFEspan client and caregiver participation.

Administrative Duties:

  • Manage scheduling and appointment setting for LIFEspan client meetings, held either virtually, by phone, or in-person.
  • Assist in general inquiries via phone and email in a professional and timely manner.
Qualifications
  • Social Service Worker diploma or an equivalent combination of education and experience.
  • Completion of a Bachelor’s Degree in Social Work, Arts, Science or equivalent is an asset.
  • Registration with the College of Social Workers and Social Service Workers is required.
  • Experience and comfort level with conversations related to death/dying (such as previous experience in a palliative care setting) and planning for this inevitability in the context of dependent adult children.
  • Experience with facilitating psycho-social and/or psycho-educational groups.
  • Experience navigating community resources and essential services.
  • Computer proficiency.
  • Knowledge of policies and programs related to income insecurity and access to housing.
  • Strong verbal/written communication and advocacy skills.
  • Knowledge and/or experience of navigating physical and developmental supports and services is an asset.
  • Previous experience working with people living with cerebral palsy and/or acquired brain injuries is preferred.
