Placements24 is seeking an experienced Community Health Navigator to join our client’s Kimberley team. The role blends direct community engagement with remote coordination to connect residents with essential health services and resources.

You will guide clients through the healthcare system, address barriers to care, and collaborate with providers and local organizations to improve health outcomes. The hybrid model enables field visits alongside administrative duties.

Qualifications
