Job summary

ParaMed is Canada’s leading provider of home health care services, delivering care to patients in their homes. The Clinical Supervisor will oversee Personal Support Workers and collaborate with the nursing team to ensure safe, ethical, and high-quality community care across our service areas.

You’ll lead staff through regular meetings, investigate patient safety incidents, and drive education, coaching and professional development while ensuring accurate clinical documentation and adherence to

Qualifications

  • Current RPN registration with the College of Nurses of Ontario and unrestricted practice.
  • Minimum 5 years' experience as a practicing Registered Practical Nurse.
  • Strong patient health assessment and community nursing knowledge.
  • Experience leading and managing a team.
  • Knowledge of clinical documentation standards.
  • Excellent organizational and teaching skills.
  • Valid driver’s license and reliable transportation.
  • Ability to obtain Vulnerable Sector Screening (VSS).

Responsibilities

  • Supervise and support Personal Support Workers to ensure practices comply with legislated standards and company policies.
  • Respond to and advise/resolve concerns from community staff, patients/families, and other team members.
  • Provide ongoing leadership through regular huddles, face-to-face meetings and one-on-ones.
  • Initiate patient safety incident investigations and document action plans.
  • Collaborate with nursing team to support clinical documentation and funder reporting guidelines.
  • Engage patients in problem solving with health teaching and relationship-building.
  • Engage staff in problem solving, education, coaching and mentoring.
  • Maintain knowledge and attitudes to provide safe, efficient service across practice areas.
