Case Manager – Toronto Community Crisis Services (Full Time – Permanent)
The Role and Opportunity
The Canadian Mental Health Association (CMHA) – Toronto is looking for a Case Manager for the Toronto Community Crisis Service (TCCS). This service provides 24/7 support to people experiencing a mental health and/or addiction crisis in northwest Toronto. This program is part of a City of Toronto police reform initiative that is evaluating the effectiveness and impact of non-police-led responses to people experiencing non-emergency crises. This program aims to reduce the harms experienced by people who have mental health and addictions challenges as well as Black, Indigenous, racialized, LGBTQ2+ and other marginalized individuals. The TCCS is governed in partnership with a Collaborative which includes the following organizations:

- Black Creek Community Health Centre
- Black Health Alliance
- Canadian Mental Health Association Toronto
- Caribbean African Canadian Social Services
- Jane Finch Centre
- Rexdale Community Health Centre
- Yorktown Family Services
The Case Manager will assess and support community members who are at risk of experiencing a crisis and/or are currently experiencing a crisis in the community, with the goal of providing culturally appropriate support, such as system navigation and timely referral to appropriate services, as well as providing short-term crisis intervention work such as counselling and case management.
Salary: $54,124 – $59,147

Responsibilities
Reporting to the Senior Manager, you will provide rapid-access, short-term, community-based follow-up support to people referred from the Mobile Crisis Team. The support provided will be client-centred, trauma- and violence-informed, recovery-oriented, and culturally safe.
This Case Manager will collaborate with individuals to conduct comprehensive needs assessments and develop service plans that incorporate individuals’ goals, values, skills, resources, strengths and service requirements. The Case Manager will engage and support individuals to access resources and navigate services in the community. The Case Manager will provide supportive counselling, health promotion and education, skill-building, problem-solving and advocacy. The Case Manager will work within the context of CMHA’s mission and strategic directions.
The Case Manager will participate in community outreach and liaison with key stakeholders and partners and maintain trusting, respectful and collaborative relationships with clients, families, colleagues, referral sources, service providers and the public.
The Case Manager will maintain accurate and timely clinical and non-clinical records and actively participate in developing, implementing, and evaluating the Toronto Community Crisis Service through meetings, training, strategic planning and evaluation initiatives.
Experience & Education Required
The ideal candidate will have at minimum an Undergraduate degree/diploma in Social Services or equivalent. The ideal candidate will have combination of education and working and/or living experience of community mental health and addictions, along with a strong knowledge of clinical and crisis assessments and intervention strategies. The candidate will have demonstrated skill in client-centred and recovery-oriented care.
The candidate will have a sound knowledge of crisis intervention and prevention strategies, addictions and harm reduction, recovery-oriented approaches, trauma- and violence-informed care, skill development and counselling.
The candidate will bring an understanding and knowledge of working with persons with mental health and/or addictions issues as well as an understanding of the intersectionality of systemic barriers and social determinants of health. The candidate will require strong knowledge of systemic and structural factors (SDoMH) that impact Black, Indigenous, racialized and LGBTQ2S+ communities. The candidate will have knowledge and/or experience of community services in the northwest Toronto area including Black Creek, Downsview, Rexdale, and Humber neighbourhoods.
The candidate will possess excellent communication, interpersonal and problem-solving skills and the ability to work as part of an interdisciplinary team. The candidate will have proficiency in Microsoft Office applications. Fluency in a second language and experience working with diverse communities is considered an asset.
Access to a car and a valid Driver’s Licence is required.
